  • A notation in the visit notes stating that there are no 1st or 2nd degree relatives with a history of cancer, or a statement to this effect would also meet the measure.                                                                                                                                                             In the physicians note under family history it stated negative for malignancy. Would this meet the measure.

@smoul Thanks for asking this, Shelly. If the physicians note does not specifically address 1st and 2nd degree relatives, it is not sufficient to meet the measure. Perhaps there is a patient intake form that addresses all 1st and 2nd degree relatives that the patient filled out during one of their visits? If so, this would be sufficient to consider the measure met. If the above statement that you shared is the only documentation of family history that you find, then you would need to answer No to 1st and 2nd degree relatives, and No blood relatives noted with cancer.
